Continuous Glucose Monitoring (CGM) and Blood Pressure (BP) monitoring when indicated.

Medication adjustments and deprescription when needed.

$400/month: For the duration of this provider-led program. Recommended duration- 4 months. This includes:

Comprehensive Consultation
Comprehensive Medical Consultation: You start with a private 90-minute session with me, Jessica Johnson, FNP-BC, ARNP, CPT a board-certified nurse practitioner, certified personal trainer, and metabolic health coach. Together we will build a custom health plan designed around your specific goals and needs.
Meal Planning Support
Nutrition is a vital component of health and weight management. The goal is a nutrition plan that is effective and one you can stick to. You will have access to resources to save you time and reduce decision fatigue. Quick start the change process with daily recipes, shopping lists, restaurant guides and more.
Medication Management
& Supplements
Medication management and deprescription: For those currently on medications with a goal to reduce or come off of them, I will work with you to make adjustments. As your insulin sensitivity and overall health improve through personalized lifestyle medicine, we’ll safely adjust your medications based on your progress.​
